There are three main components in it:
an installer which prepares the system for the final application
a framework to bind and load a library at runtime,
the application itself.

There is no one best practice, but there are some patterns that might be worth looking at.
If you are distributing for the desktop:
Use Click and/or NSIS.
Use Click to build an executable that can be run by double clicking.
Use NSIS to build an installer that will run under WINE or similar.
If you are distributing for the web:
Use the Qt WebKit bindings.
Use the system's web server to host your application.
Use a file transfer protocol to send the binary and any libraries.
For other platforms (e.g. the Raspberry Pi), then:
Use QtWebKit or similar to build an executable that can be run by double clicking.
For small-scale, embedded systems, then:
Use one of the Qt libraries in a QNX or similar embedded environment.
